To Enroll a student at Perquimans County High School, please follow these procedures:
In order to better serve you, we are requesting the following documents and information. A new student will not be admitted without all of the essential documents listed below:
Student Withdrawal Process
Transcripts
There is no processing fee for enrolled student transcripts. There is a $3 charge per transcript processing fee for former students. Please click on Transcript Request Form
Official Transcripts are signed by a school official, embossed with the school’s seal ad placed in a sealed envelope.
Unofficial Transcripts are not signed or sealed.
To Enroll a student at Perquimans County High School, please follow these procedures:
- Call 252-426-5741 and make an appointment to register your child & pick up a Student Enrollment & Registration Form Packet.
- Only the custodial parent or guardian can enroll a child. No individual can enroll a student unless he or she has legal custody of the student.
In order to better serve you, we are requesting the following documents and information. A new student will not be admitted without all of the essential documents listed below:
- Proof of Residence – A recent bill from an electric company, rental receipt indicating name and address on the receipt or a mortgage document will be accepted.
- Proof of Legal Guardianship if other than custodial parent enrolling the child. Only the parent or guardian can enroll a child and must have legal custody.
- All legal documentation/Court Custody papers must accompany enrollment.
- Birth Certificate, Social Security Card and Immunization Record
- Recent Report Card, Transcript, Withdrawal Form and Discipline Record.
- Current IEP (Individualized Education Plan) or 504 Plan (if applicable). Must be from previous school.
- Home Language Survey Form
- School Bus Transportation Form
- New Student Enrollment Form
Student Withdrawal Process
- A parent or legal guardian must physically come in to school and complete the Withdrawal Form.
- The student must return their IPAD with charger, books, pay all fees outstanding for library books and cafeteria fees (may have a refund). Lockers must be cleaned out.
- Withdrawal is not final until the above requested items are completed.
